The short version: many people can now buy life insurance without a nurse visit, blood draw, or urine test. Healthy applicants can often get full-price coverage through accelerated underwriting. People with health issues can use simplified or guaranteed issue policies, which cost more and offer less coverage.
Three types of no-exam life insurance
| Type | Health questions | Typical coverage | Cost |
|---|---|---|---|
| Accelerated underwriting | Full application, records checked | Often up to $1M+ depending on age | Same as fully underwritten |
| Simplified issue | A short list of yes/no questions | Usually up to a few hundred thousand, lower for seniors | Higher |
| Guaranteed issue | None | Usually $25,000 or less | Highest per dollar of coverage |
Limits vary by insurer, age, and state.
Accelerated underwriting
This is the best option if you're in good health. Instead of an exam, the insurer checks databases it's allowed to use with your permission, such as prescription history, the MIB (a shared record of past insurance applications), and your driving record. If everything lines up, you can be approved in days, sometimes minutes, at the same rates as a traditional policy.
If something in your records needs a closer look, the insurer may ask for an exam after all. That doesn't mean a denial. It just means standard underwriting.

Simplified issue
Simplified issue policies ask a short set of health questions and skip the exam. They're designed for people with manageable health conditions who might not qualify for the best rates, or who want a fast, simple process. Expect higher premiums and lower maximum coverage than a fully underwritten policy.
Guaranteed issue
Guaranteed issue policies accept everyone in the eligible age range, typically around 45 or 50 to 80 or 85, with no health questions. The tradeoffs are real:
- Coverage is small, usually enough for funeral costs.
- Premiums are the highest per dollar of coverage.
- Most have a graded death benefit: if you die of natural causes in the first two or three years, beneficiaries typically get back your premiums plus some interest instead of the full amount.
Guaranteed issue is a last resort for people who can't qualify elsewhere. Many people with health conditions qualify for a simplified issue final expense policy, which usually pays the full benefit from day one.
How to choose
- Healthy and under 60? Start with accelerated underwriting. You'll get the best rate with no exam.
- Some health issues? Compare fully underwritten and simplified issue. Sometimes taking the exam gets you a much better price.
- Serious health issues or over 75? Look at simplified issue final expense first, then guaranteed issue.
